You may be worried about menstrual pain or heavy periods. Or, you might have been referred to a paediatric gynaecologist because your periods are irregular or absent. Painful, irregular and heavy periods are not unusual in the years after your first cycle (did you know it is called ‘menarche’?). Up to 40% of young girls will mention irregular periods throughout that first year. For some, it can continue for up to 4 years.

We know that it can take on average 7 to 10 years to diagnose endometriosis and that painful periods are one of the typical symptoms. So you may wonder if serious period pain in a 15-year-old girl means that she needs to be checked for endometriosis.
Most adolescents who present with painful periods have no underlying condition. Anti-inflammatory drugs are then often the recommended treatment, or alternatively hormonal suppression.
If that type of treatment fails, your gynaecologist may plan further evaluation. At least two-thirds of adolescent girls with chronic pelvic pain or painful periods (unresponsive to hormonal therapy or drugs) will be diagnosed with endometriosis.
The procedure used to diagnose endometriosis is a ‘diagnostic laparoscopy’.

Precocious puberty can affect your child’s confidence and wellbeing. In girls, precocious puberty is defined as the development of pubertal characteristics at the age of 8 or younger. It is recommended to see your paediatric gynaecologist and to seek advice if this is the case.
Delayed puberty is when girls do not show obvious development (pubic hair, breast development) by 12-14 years of age, or if there is a delay in the onset of menstruation after 16 years old.
It is also recommended to seek professional help if things seem out of order, for example having a period before breast development occurs.
